1,25-dihydroxyvitamin D (calcitriol)
This test measures 1,25-dihydroxyvitamin D, the biologically active form of vitamin D. It is used to investigate calcium imbalance, kidney disease and...

SelectedThe Advanced HRT Optimisation Profile is a comprehensive follow-up blood panel designed for individuals already on hormone replacement therapy (HRT) or considering optimisation beyond baseline treatment. It goes deeper than standard testing by assessing hormone balance alongside thyroid performance, insulin resistance, cardiovascular risk, inflammation, and nutritional status — all of which influence how well hormone therapy works.
This profile helps clinicians refine dosing, identify barriers to symptom improvement, and ensure long-term safety while maximising therapeutic benefit.
Oestradiol
Progesterone
Testosterone (Free and Total)
Sex Hormone Binding Globulin (SHBG)
DHEA-S
Cortisol (AM)
These markers evaluate hormone balance, adrenal function, androgen status, and therapy response.
Thyroid Stimulating Hormone (TSH)
Free Thyroxine (Free T4)
Free Tri-iodothyronine (Free T3)
Thyroid health significantly affects metabolism, energy levels, and hormone effectiveness.
Fasting Glucose
HbA1c
Fasting Insulin
HOMA-IR (calculated)
Insulin resistance can blunt hormone response and contribute to weight gain or fatigue.
Lipid Profile (Total Cholesterol, HDL, LDL, Triglycerides)
Apolipoprotein A1
Apolipoprotein B
Lipoprotein(a)
These markers provide a deeper understanding of cardiovascular risk beyond standard cholesterol testing.
High-Sensitivity C-Reactive Protein (hs-CRP)
Homocysteine
These markers reflect systemic inflammation, vascular health, and biological ageing processes.
Vitamin D
Vitamin B12
Folate
Ferritin
Magnesium
Zinc
Nutrient status plays a critical role in hormone metabolism, mitochondrial function, and energy levels.
Hormone therapy outcomes depend on multiple systems, not just reproductive hormones. Without advanced monitoring, issues like insulin resistance, inflammation, or thyroid dysfunction may limit results.
This panel is recommended if you:
• Are already on HRT and want optimisation
• Have persistent symptoms despite treatment
• Experience fatigue, weight gain or mood changes
• Want precise hormone balancing
• Are monitoring long-term safety
• Are pursuing longevity or preventative health strategies
Expanded benefit explanation
By evaluating hormones alongside metabolic and cardiovascular factors, this panel allows true optimisation rather than symptom-based adjustments alone.
We offer two UK-wide options:

Where possible, attend your blood test between 7am and 12pm. Please wait until any short-term illness or infection has fully resolved before testing. Avoid intense exercise for 24–48 hours beforehand, as this can affect certain markers.
Fasting is not always required, but some tests may recommend it. If fasting is advised, avoid food for 8–12 hours before your appointment and drink water only. Stay well hydrated, as this helps with sample collection and accuracy.
Continue prescribed medications unless advised otherwise by your clinician. Avoid vitamin, mineral, or biotin supplements for at least 24–48 hours before your test, as these can interfere with results. If you are unsure about any medication or supplement, please let us know before your appointment.
For hormone-related tests, timing within your menstrual cycle may be important. If relevant, follow any specific guidance provided on your test page.
If you have any questions or special circumstances, our team is happy to advise before your appointment.
