Details about Enhanced Liver Fibrosis

What is the Enhanced Liver Fibrosis (ELF) test?

The Enhanced Liver Fibrosis (ELF) test is a specialised blood test that measures a combination of biomarkers associated with liver fibrosis, the process of scarring that occurs when the liver is repeatedly injured or inflamed.

The test analyses three key markers involved in extracellular matrix turnover and fibrotic activity: Hyaluronic Acid (HA), Procollagen III N-terminal peptide (PIIINP), and Tissue Inhibitor of Metalloproteinase-1 (TIMP-1). These are combined into a single ELF score.

Unlike liver biopsy, the ELF test is non-invasive, reproducible, and suitable for monitoring liver health over time.

Why do I need an Enhanced Liver Fibrosis test?

Liver fibrosis often develops silently and may progress to cirrhosis before symptoms appear. Early detection allows intervention before irreversible damage occurs.

You may need an ELF test if you:

  • Have non-alcoholic fatty liver disease (NAFLD or MASLD)
  • Have metabolic syndrome or insulin resistance
  • Have type 2 diabetes
  • Drink alcohol regularly or heavily
  • Have chronic viral hepatitis
  • Have abnormal liver function tests
  • Need fibrosis risk assessment without biopsy

Conditions associated with liver fibrosis include:

  • Fatty liver disease
  • Chronic hepatitis B or C
  • Alcohol-related liver disease
  • Obesity-related liver inflammation
  • Long-term medication-related liver injury

Benefits of the Enhanced Liver Fibrosis test

  1. Non-invasive fibrosis assessment
    No biopsy required.
  2. Detects early and advanced fibrosis
    Identifies risk before cirrhosis develops.
  3. Clinically validated liver score
Used in hepatology and metabolic medicine.
  4. Suitable for monitoring progression
    Tracks fibrosis changes over time.
  5. Useful in fatty liver disease
    Strong role in NAFLD/MASLD assessment.
  6. Supports risk stratification
    Helps guide referral and management.
  7. Simple blood test
    Minimal patient burden.
  8. Avoids biopsy-related risks
    No pain or procedural complications.

Step by step – how the test works

You can access Enhanced Liver Fibrosis testing UK-wide through two options:

  1. Clinic appointment
Attend your nearest UK clinic where a healthcare professional will take your blood sample.
  2. At-home or hotel visit (£60 extra)
A qualified phlebotomist comes to your home or hotel, so you don’t need to travel.

Your blood sample is analysed in an accredited UK laboratory and an ELF score is calculated.

Your results explained

Low ELF score – minimal fibrosis

Indicates little or no liver scarring.

Typical UK interpretation:

  • ELF < 7.7

Moderate ELF score – developing fibrosis

Suggests ongoing fibrotic activity and increased risk.

Typical UK interpretation:

  • ELF 7.7 – 9.8

High ELF score – advanced fibrosis

Indicates significant liver scarring with risk of progression to cirrhosis.

Typical UK interpretation:

  • ELF > 9.8

Cut-offs may vary slightly by clinical guideline and population.

How to Prepare for Your Test

Follow these guidelines for accurate results

Sample Timing

Where possible, attend your blood test between 7am and 12pm. Please wait until any short-term illness or infection has fully resolved before testing. Avoid intense exercise for 24–48 hours beforehand, as this can affect certain markers.

Fasting & Hydration

Fasting is not always required, but some tests may recommend it. If fasting is advised, avoid food for 8–12 hours before your appointment and drink water only. Stay well hydrated, as this helps with sample collection and accuracy.

Medications & Supplements

Continue prescribed medications unless advised otherwise by your clinician. Avoid vitamin, mineral, or biotin supplements for at least 24–48 hours before your test, as these can interfere with results. If you are unsure about any medication or supplement, please let us know before your appointment.

Hormones & Menstrual Cycle

For hormone-related tests, timing within your menstrual cycle may be important. If relevant, follow any specific guidance provided on your test page.

If you have any questions or special circumstances, our team is happy to advise before your appointment.
