Details about Erythrocyte Sedimentation Rate (ESR)
ESR Blood Test | Inflammation Marker | London Blood Tests
Measure inflammation with our ESR blood test for £55. UKAS-accredited testing, fast results, and optional GP consultation available.
The Erythrocyte Sedimentation Rate (ESR) Test is a simple blood test used to detect inflammation in the body. It helps identify inflammatory, autoimmune, or infectious conditions and monitor disease activity. Testing is performed in a UKAS-accredited laboratory, with fast results and an optional GP consultation.
What is an ESR Blood Test?
The Erythrocyte Sedimentation Rate (ESR) measures how quickly red blood cells settle at the bottom of a test tube over one hour. When inflammation is present, proteins in the blood cause red cells to clump together and sink faster, resulting in a higher ESR value.
ESR does not diagnose a specific condition, but it is a valuable marker for detecting and monitoring systemic inflammation. It is commonly used alongside other tests such as CRP, full blood count, and autoimmune markers to build a clearer clinical picture.
Because ESR reflects ongoing inflammatory activity, it is particularly useful for monitoring chronic inflammatory or autoimmune diseases over time rather than detecting acute infection alone.
Benefits of the ESR Test
-
Detects General Inflammation
Identifies whether inflammation is present anywhere in the body. -
Supports Autoimmune Disease Monitoring
Commonly used to monitor conditions such as rheumatoid arthritis, lupus, and vasculitis. -
Assists in Infection Investigation
Helps differentiate inflammatory or infectious causes of unexplained symptoms. -
Monitors Disease Activity Over Time
Rising or falling ESR values can reflect worsening or improving inflammation. -
Simple and Cost-Effective
A low-cost blood test requiring minimal preparation. -
Complements Other Inflammatory Markers
Often used alongside CRP to distinguish acute vs chronic inflammation. -
Optional GP Consultation for Interpretation
Results can be reviewed with a GP to determine next steps or further testing.
Why Do I Need an ESR Blood Test?
You may benefit from an ESR test if you experience symptoms that suggest inflammation, including:
-
Persistent fatigue or malaise
-
Joint pain, stiffness, or swelling
-
Muscle aches
-
Fever of unknown origin
-
Unexplained weight loss
-
Chronic headaches or visual disturbances
-
Suspected autoimmune or inflammatory disease
ESR testing is also useful for:
-
Monitoring known autoimmune or inflammatory conditions
-
Assessing response to treatment
-
Investigating unexplained symptoms when initial tests are inconclusive

Your ESR Test Results – Explained
ESR is reported in millimetres per hour (mm/hr).
Typical adult reference ranges:
-
Men: < 15 mm/hr
-
Women: < 20 mm/hr
-
Older adults: slightly higher values may be normal
Interpretation:
-
Normal ESR: little or no systemic inflammation
-
Mildly elevated: possible low-grade or chronic inflammation
-
Markedly elevated: active inflammation, infection, or autoimmune disease
ESR results should always be interpreted alongside symptoms and other blood tests, as many conditions can influence the result.

WhatsApp UsHow to Prepare for Your Test
Follow these guidelines for accurate results
Sample Timing
Where possible, attend your blood test between 7am and 12pm. Please wait until any short-term illness or infection has fully resolved before testing. Avoid intense exercise for 24–48 hours beforehand, as this can affect certain markers.
Fasting & Hydration
Fasting is not always required, but some tests may recommend it. If fasting is advised, avoid food for 8–12 hours before your appointment and drink water only. Stay well hydrated, as this helps with sample collection and accuracy.
Medications & Supplements
Continue prescribed medications unless advised otherwise by your clinician. Avoid vitamin, mineral, or biotin supplements for at least 24–48 hours before your test, as these can interfere with results. If you are unsure about any medication or supplement, please let us know before your appointment.
Hormones & Menstrual Cycle
For hormone-related tests, timing within your menstrual cycle may be important. If relevant, follow any specific guidance provided on your test page.
If you have any questions or special circumstances, our team is happy to advise before your appointment.
